Community Nursing Care
At Genesis Tasmania, our Community Nursing Care service is dedicated to providing comprehensive and holistic care to individuals with disabilities in their home environment. Our registered and highly skilled nurses offer a wide range of medical, rehabilitative, and supportive care to ensure our participants’ optimal health and well-being.
Our services include but are not limited to, wound care, medication management, chronic disease management, post-hospitalisation care, palliative care, and assistance with daily self-care activities. We believe in a person-centred approach, tailoring our services to meet each participant’s needs and preferences. We collaborate with other healthcare professionals, caregivers, and participants to develop a holistic, efficient care plan that respects the participant’s wishes.
Our Community Nursing Care service aims to enhance the health outcomes of individuals with disabilities while maximising their comfort and dignity in their homes. This service fosters independence, encourages self-management, and ensures continuity of care, supporting our participants’ overall well-being and quality of life.
